I have found that you can get away with forwarding: UDP 6073

and tcp ports 2300-2400. That's all you should need but you may have problems if you don't forward the rest of the ports microsoft sugjests.

Also you can check http://www.puffinsoft.com they hav some useful info on connecting to dircetplay games through a firewall/router.
